The standard approach through the popliteal fossa to the tibial attachment of the posterior cruciate ligament (PCL) can be time-consuming and is regarded as hazardous. The simplified approach does not transect or expose neurovascular structures as in other procedures. With dissection on the medial border of the medial head of the gastrocnemius and by lateral retraction, the posterior capsule is safely exposed. The approach is ideal for internal fixation of an avulsed tibial attachment of the PCL.
